About Your Upcoming Appointment
Our office policy at Elm Family Dental is to treat all patients with respect and provide a warm and welcoming environment where everyone receives the highest quality of dental care. Our aim is to make sure every patient leaves us knowing they have gotten everything they need to maintain better dental health.
We are currently welcoming new patients. Please call us to schedule and reserve an appointment. If you must cancel an appointment, please contact us
as soon as possible to reschedule and avoid missed appointment fees.
What to Bring for Your First Visit
- Insurance information card
- Photo identification
- Any medical record pertaining to your visit
- Please arrive 15 minutes early to fill out the necessary paperwork
Insurance
Please bring your dental insurance information with you on your first appointment. Our staff will gladly file the necessary insurance claims to give you the maximum benefit your plan allows. Feel free to contact us
at any time if you have any questions about the details of your insurance plan.
Payment
We expect payment for your dental services when your services are provided unless prior arrangements have been made. This includes co-payments, deductibles, and any amount your insurance company doesn't cover.
